Field Case Manager: Homeless Outreach & Housing Access

The People Concern

The People Concern's Santa Monica C3 Team is seeking a case manager to join our multidisciplinary street outreach team. This role supports a City of Santa Monica strategy to identify, engage, and connect homeless individuals to interim or permanent housing and supportive services. The Case Manager will help clients develop individualized service plans, enroll in benefits, submit housing applications, and access medical and behavioral health treatment, while prioritizing wellness, recovery and #J-18808-Ljbffr The People Concern
