Hospice Registered Nurse (RN) Case Manager

$40 - $46 per hour

We are partnering with a well-established, multi-state hospice provider known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings. This organization combines a supportive, team-oriented environment with the resources and stability of a larger healthcare platform.
Pay and Schedule

$40-$46/hour

Monday-Friday, 8:00am-4:30pm
On-call rotation approximately every 5 weeks
Includes weekend coverage (Saturday and Sunday)
$6/hour hold rate plus time-and-a-half for visits during call

Caseload of 12-14 patients (maximum 16-17)
Approximately 25 visits per week (about 5 per day)
60% of visits are facility-based, supporting more efficient scheduling
Centralized triage support after hours
Scheduler support for daily planning
Structured onboarding and training program

Responsibilities

Manage hospice patient caseload across home and facility settings
Perform patient assessments and develop individualized care plans
Provide skilled nursing care and symptom management
Coordinate with interdisciplinary team members
Maintain documentation using Homecare Homebase
Participate in on-call rotation

Territory

Coverage includes York and Lancaster areas
Patients are assigned geographically to support efficiency
Typical day includes approximately one hour of drive time
Mix of facility and home-based hospice care

Requirements

Active RN license
At least one year of clinical experience (acute care, home health, or hospice preferred)
Valid driver's license and reliable transportation
