Responsibilities Manage the plan of care for a caseload of patients, assuming 24-hour responsibility for patient caseload. Serve as the liaison with the multidisciplinary care team. Plan care management achieved through coordination of care with physicians, nursing staff and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources.
