About the Job
- Provide skilled nursing and personal care according to the client’s individualized care plan.
- Perform ventilator, tracheostomy, and G-tube care.
- Monitor and document the client’s condition and report changes promptly.
- Administer medications and tube feedings as ordered (RN/LPN).
- Maintain accurate documentation.
- Follow infection control and safety protocols.
- Communicate effectively with families and the healthcare team.
