Responsibilities Assess patients' physical abilities and functional status. Develop personalized treatment plans to improve mobility, strength, and independence. Provide therapy in an outpatient setting and adjust treatments as needed. Collaborate with healthcare teams to ensure comprehensive care. Educate patients and monitor progress throughout the course of treatment. Participate in discharge planning and maintain documentation of patient progress.
