We are seeking a compassionate, clinically skilled, and highly motivated Registered Nurse (RN) Case Manager to join a collaborative healthcare team dedicated to providing comprehensive, patient-centered care to older adults!
This is an excellent opportunity for an experienced RN who enjoys building meaningful relationships with patients, coordinating complex care needs, and making a positive impact on the health and well-being of seniors.

The position includes three on-site days and two mobile days per week, allowing you to provide both clinic-based care and direct support to patients in their homes.
The ideal candidate has recent nursing experience involving ongoing comprehensive assessments, care coordination, and complex clinical decision-making across a diverse patient population.
