Function The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Qualifications Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services. Ability to remain calm in emergency or crisis situations. Graduate of an APTA‑approved physical therapy program. At least one year of experience as a physical therapy aide. Current license from the state Board of Physical Therapy Examiners. Essential Functions and Responsibilities Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. Review patients’ functional needs and adjust care plans to ensure optimal outcomes. Design and implement a plan of care for patients based on a thorough assessment and incorporate participation by significant others when appropriate. Conduct therapy practice within defined standards of care. Direct support staff to achieve patients’ functional goals. Communicate effectively with other professionals and support staff. Practice hospital and departmental policies and procedures, quality improvement, safety, environmental, and infection control standards. Document evaluations, treatment goals, plans; maintain prescriptions and signatures. Enhance professional growth of self and others through participation in educational programs, in‑service meetings and workshops. Stay current on professional literature. Provide direction and leadership to staff and serve as a resource to promote physical therapy in the community. Demonstrate reading, writing, and speaking English sufficient for job requirements. Make independent decisions and solve practical problems. Listen actively and evaluate social situations fairly. Work independently with integrity, flexibility; collaborate with residents, personnel, and support agencies. Comply with community policies for workplace safety, including infection control and emergency procedures. Comply with all company policies regarding resident rights, abuse prevention, reporting, and confidentiality (HIPAA). Demonstrate knowledge of Employee Handbook and comply with its policies. Perform other related duties as assigned.
