$47,050 - $70,575 Annually This position is a Pay Band C5 Posting Details The Consumer and Elder Rights Division (CERD) within the New Mexico Aging & Long-Term Services Department (ALTSD) provides accessible, integrated services to older adults, adults with disabilities and caregivers to assist them in maintaining their independence, dignity, autonomy, health, safety, and economic well-being, thereby empowering them to live on their own terms in their own communities as productively as possible. We envision lifelong independence and healthy aging. Our guiding principles are to protect the safety and rights of those we serve; promote personal choice and self-determination; treat all persons with respect, embracing cultural diversity; encourage collaborative partnerships; and provide fiscally responsible services. CERD oversees the Aging and Disability Resource Center (ADRC), State Health Insurance Assistance and Information Program (SHIP), and Senior Medicare Patrol (SMP). *APPLICATIONS SUBMITTED WITHOUT WORK EXPERIENCE OR A RESUME WILL NOT BE CONSIDERED. Why does the job exist? The purpose of this position is to identify, coordinate, review, organize, and evaluate the daily living needs of individuals calling the Aging & Disability Resource Center (ADRC). The position will provide assistance to those who do not have the ability to resolve problems or access services on their own. The main purpose is to help consumers identify and understand their needs and to assist them in making informed decisions about appropriate long-term service and support choices in the context of their personal needs, preferences, values and individual circumstances. Resource Options Counseling also involves assisting people in their understanding their health care and other benefits through SHIP, as well as helping middle aged individuals to plan ahead for their long-term care through the use of private insurance and alternative living arrangements.
How does it get done? Provide over the phone and walk-in benefit counseling.
Conduct an assessment to address and identify the various needs of the clients. Provide assistance in complex cases by providing information, engaging in identifying clients needs, and offering appropriate information in order to increase access to social service programs.
Provide individual client counseling regarding Medicare, Medicaid, health and long term care insurance, public benefits and appropriate community programs and services. Incumbent will review eligibility determinations for such social service programs.
Provide information and assistance with two (2) or more client needs, determine the actions previously taken, assistance previously provided, and results of the actions and assistance in order to refine the list of possible resources and assistance that may be needed. Identify appropriate available services and supports by providing enough information and referral about each organization to help inquires make informed choices.
Complete current client contact records.
Complete the record of the current contact by obtaining additional information, if necessary, and document assessment, client demographic information, outcomes, referrals, client notes and note follow up efforts in ADRC data base.
Who are the customers? The general public, elderly and disabled population, and caregivers.
Ideal Candidate The ideal candidate must be professional, courteous, civil, respectful and must provide open, honest and thorough communication.
Minimum Qualification High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
Employment Requirements Pre-employment state and federal criminal background investigation is required and is conditional pending results. Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within 90 days of employment.
Working Conditions Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds. Occasional travel to regional offices.
