Job Summary:

We are seeking a highly skilled and compassionate Home Care Team Leader to join our team in Bay Minette, Alabama. As a Home Care Team Leader , you will be responsible for overseeing and leading a team of home care professionals to provide exceptional care and support to our clients. Responsibilities:

• Lead a team of home care professionals to provide medical and non-medical services to clients in their homes

• Conduct initial assessments and develop individualized care plans for clients

• Supervise and evaluate team members to ensure compliance with company policies and procedures

• Collaborate with healthcare professionals, clients, and families to ensure continuity of care

• Provide education and training to team members on new procedures, protocols, and best practices

• Maintain accurate records and documentation of client care and team member performance

Requirements:

• Bachelor's Degree in Nursing, Health Care Administration, or related field

• Minimum 2+ years of experience in home care, hospice, or healthcare management

• Strong leadership and communication skills

• Ability to work independently and make sound decisions

• Proficiency in electronic medical records and other healthcare software

• Current CPR certification

• Knowledge of Medicare and other payor sources

• Ability to pass a criminal background check
