Responsibilities of Hospice Nurse Case Manager include: Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Supervise other home care staff as necessary. Qualifications of a Hospice Nurse Case Manager include: Must possess an active RN license in both VT and NH. ASN required, BSN or MSN preferred. Weekend and on-call rotation required. Qualifications Education Required Associates or better in Nursing. Preferred Bachelors or better in Nursing.
