Field-Based Community Health Advocate
Constella Care
Constella Care is seeking a Community Health Worker to serve in Kentucky. You will be the on-the-ground representative for members on your panel, performing outreach, home visits, and crisis response while coordinating access to housing, benefits, and community resources. You’ll use an EHR daily and work closely with the clinical team to ensure members stay connected to care.
