Clinical Staff Pharmacist – ED, 7 on 7 off, 10K Bonus (920491) Job Summary This role will work 7 on and 7 off, 10‑hour shifts from 11 am to 9:30 pm with full‑time benefits. ED or ICU experience preferred. Texas license with TSBP within 30 days. (ACPE‑IV) Sterile Compounding (IV) certification accredited by ACPE upon hire. (BLS) Basic Life Support certification. (AHA/ARC) Advanced Cardiac Life Support within 180 days for inpatient positions. Job Duties Perform job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations. Perform all computer order entry accurately, thoroughly and timely. Review and update medication profiles (therapies, allergies, height, weight, etc.) and be cognizant of patient charges. Review medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seek clarification if necessary and document interventions (i.e., ADR). Consistently work (multi‑tasks) and problem‑solve independently and effectively with minimal supervision. Take charge when necessary and is looked to for direction. Make the right decisions and use good judgment. Be cross‑trained and multifunctional maintaining competency in multiple areas. Enforce the formulary system and follow departmental policy and procedure when acquisition of non‑formulary product is judged necessary. Participate in cost‑effective use and selection of pharmaceuticals. Communicate drug information verbally and in writing to physicians, nurses, etc. in a courteous and professional manner. Assist in patient/family education and counseling regarding drug therapy and side effects. Also, effectively document clinical activities, demonstrating cost savings and concisely conveying key information required for follow‑up by fellow pharmacists. Strive to improve patient satisfaction, as measured by Press Ganey scores. Manage technicians and all other supportive personnel. Delegate appropriate reasonable tasks and validate accuracy of medications intended for distribution from the IV lab, central pharmacy or decentralized automated dispensing machines. Audit and sign out all controlled substances in accordance with state/federal guidelines and organizational policies. Investigate and correct discrepancies and unusual usage patterns/trends. Actively enhance personal growth and development by participating in educational programs, in‑services and meetings. Actively participate in new employee orientation and training. Willingly take on additional projects as needs develop. Actively participate in the clinical program. Support P&T initiatives by monitoring target drugs for protocol discontinue, substitution, IV to PO, etc. Collect applicable data/measure and communicate opportunities for improvement. Perform therapeutic monitoring of specified drug therapies and recommend dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations. Duties performed may include one or more of the following core functions: a) Directly interacting with or caring for patients; b) Directly interacting with or caring for human‑subjects research participants; c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or d) Regularly maintaining, modifying, releasing or similarly affecting human‑subjects research records. Perform other duties as assigned.
