Support safe patient mobility and transitions, assisting with admission, transfer, and discharge processes to ensure a seamless care experience. Communicate key observations and changes in physical or emotional condition promptly to the nursing team to facilitate timely clinical interventions. Document patient care activities accurately and thoroughly to support the care team and maintain reliable records. Maintain a clean, orderly, and safe patient environment while keeping medical equipment and supplies ready for immediate clinical use. What Minimum Requirements You’ll Need BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Nurse Aide credentialed from the Wisconsin Nurse Aide Registry obtained prior to hire date or job transfer date required. Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date. Additional certs may be required by specific job locations. Education High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
