- Lead and manage the social services department, overseeing daily operations and ensuring compliance with regulatory requirements and company policies
- Conduct comprehensive assessments to identify residents' social, emotional, and psychological needs, and develop individualized care plans to address those needs
- Collaborate with care teams, healthcare professionals, and family members to ensure a coordinated approach to care and effective communication
- Coordinate and facilitate discharge planning, ensuring a smooth transition from hospital to home or alternative care settings
- Develop and maintain relationships with community organizations, home health agencies, and other healthcare providers to ensure access to necessary resources and services
- Monitored residents' progress and adjust care plans as needed to achieve optimal health, physical function, and independence
- Master's degree in social work or related field
- Previous experience in Nursing Home Facility/Long Term Care
