Registered Nurse - Case Management - Hospital

Job Description

Job Summary:
The Hospital Case Management Registered Nurse is responsible for coordinating patient care plans, ensuring appropriate resource utilization, and facilitating discharge planning to optimize patient outcomes and hospital efficiency.

Responsibilities:

Assess patients' needs to develop individualized care plans.
Collaborate with healthcare team members to coordinate treatment and discharge instructions.
Monitor patient progress and adjust care plans as necessary.
Ensure compliance with hospital policies, regulations, and documentation standards.
Educate patients and families on post-discharge care and resources.
Document all patient interactions and care activities accurately in electronic health records.

Qualifications:

Hold a valid MD or Compact Nursing license in good standing.
Possess at least 1 year of hospital case management experience.
Demonstrate strong knowledge of healthcare regulations and documentation.
Exhibit excellent communication and interpersonal skills.
Be adaptable to float within scope of practice when required.
Have current certifications in case management or related fields (preferred).
