Job Title: Physical Therapist — Home Health Location: Sterling Heights, MI — Field-based Type: Full-Time | On-site Positions Available: 1 Job Summary We are seeking a licensed Physical Therapist for home health field positions across Macomb, Oakland, and Wayne counties in Michigan. The PT will evaluate and treat patients in their homes, developing and implementing individualized therapy plans to restore function, improve mobility, and prevent rehospitalization. Key Responsibilities Conduct comprehensive physical therapy evaluations and develop individualized treatment plans Deliver skilled PT interventions in the home setting focused on mobility, strength, and functional independence Educate patients and families on home exercise programs, fall prevention, and safe mobility Complete documentation accurately in compliance with Medicare home health standards Collaborate with nursing, OT, and social work staff on patient care plans Participate in discharge planning and transition of care activities Requirements Active Michigan PT license required Minimum 1 year of home health or outpatient PT experience preferred CPR/BLS certification required Valid Michigan driver's license and reliable transportation required Strong clinical assessment and patient education skills #J-18808-Ljbffr The Employee Connect
