Licensed Vocational Nurse

Under the direction and supervision of the Home Care Manager, the Licensed Vocational Nurse provides direct nursing services to participants.

Duties/Responsibilities:

1. Provides skilled nursing care to all participants including but not limited to administration and monitoring medications, checking vital signs and monitoring participant's changes.
2. Supervises, trains, and monitors geriatric aides. Teaches participants and family members regarding participant needs at home.
3. Communicates effectively with all team members, participants, and families. Participates in interdisciplinary team planning and problem-solving.
4. Other related duties as assigned.

Qualifications (Knowledge, Skills, Abilities):

1. Graduate of an accredited LVN program.
2. Possess a current California LVN license.
3. Interest, demonstrated ability and one-year experience working with the elderly or frail population required.
4. Ability to work independently and as part of multidisciplinary team.
5. Cantonese-speaker preferred, not required.
