Community Health Navigator — Field Outreach & Care Coordination
Christian Community Health Center
Christian Community Health Center in South Holland, IL seeks a Community Health Worker (CHW) contractor to join our front-line care team. You will conduct home visits and outreach in Southside Chicago neighborhoods, perform point-of-care testing (A1c, glucose, blood pressure), and connect patients with social determinants of health resources to close care gaps.
