Clinical Pharmacist in Westerville, OH

An experienced clinical pharmacist with a specialized focus on home infusion is essential to oversee the preparation, dispensing, and management of complex intravenous (IV) and subcutaneous medications. This role serves as a pivotal member of a multidisciplinary team providing patient-centered care through sterile compounding, medication adjustment, and education.


Key Responsibilities:

• Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment compliant with state and federal regulations.
• Clinical Monitoring: Evaluate lab values, conduct pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatment plans. Provide detailed patient counseling on specialty medications.
• Patient Onboarding and Education: Instruct patients and caregivers on infusion pump usage, sterile technique maintenance, and catheter care.
• Care Coordination: Work closely with prescribing physicians, home health nurses, intake, referral, reimbursement, nursing, clinical services, and pharmacy operations teams to ensure coordinated therapy and reduce hospital readmissions.
• Regulatory Compliance: Ensure adherence to regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and related standards.
• Inventory and Shipping: Manage pharmacy inventory, including refrigerated products, and prepare medication shipments for timely, safe delivery.

Qualifications and Desired Experience:

• Minimum of 2 years' experience in infusion specialty pharmacy.
• IVIg experience strongly preferred.
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Licensed pharmacist in New York, current and in good standing.
• Proficiency in CareTend system.
