Job Title

Physical Therapist – Long-Term Care / Rehabilitation

Hilo, HI

Schedule / Job Type

Contract | Days | Full-Time | Onsite | Generally five (5) days per week, likely including one weekend day

Key Responsibilities

• Provide rehabilitation services within a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) setting
• Perform evaluations, reassessments, treatments, and clinical services consistent with licensure and scope of practice
• Develop and implement individualized treatment plans in collaboration with physicians and the interdisciplinary team
• Apply evidence-based therapeutic interventions to maximize resident function, independence, mobility, and quality of life
• Educate residents, families, and caregivers regarding treatment plans and patient progress
• Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements

Required Qualifications

• Current, active State of Hawaii Physical Therapist license in good standing — must be obtained prior to assignment start date
• Current Basic Life Support (BLS) certification
• Ability to work Hawaii State and Federal Holidays as required based on facility operations
• Willingness to float to other units or facilities based on census and staffing needs

Preferred Experience / Skills

• Minimum two (2) years of experience working in a SNF or ICF setting
• Familiarity with EMR documentation systems, including Net Health Rehab Optima and Epic
