Occupational Therapist (OTR)

The Occupational Therapist (OT) is responsible for evaluating, planning, and implementing individualized occupational therapy treatment programs for residents in a skilled nursing facility. The OT works collaboratively with physicians, nursing staff, rehabilitation professionals, residents, and families to restore function, improve independence with activities of daily living (ADLs), and enhance quality of life while complying with all federal, state, and facility regulations.

• Evaluate residents to determine occupational therapy needs and establish individualized treatment plans.

• Develop measurable goals that promote independence in activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility, cognition, and upper extremity function.

• Administer therapeutic exercises, activities, modalities, and adaptive techniques based on physician orders and clinical judgment.

• Assess and recommend adaptive equipment, splints, braces, and environmental modifications to maximize resident safety and independence.

• Educate residents, family members, and caregivers on treatment plans, home exercise programs, adaptive equipment, and safety techniques.

• Document evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries accurately and within required timeframes.

• Participate in interdisciplinary care plan meetings and collaborate with nursing, physicians, social services, and other rehabilitation team members.

• Monitor resident progress and modify treatment plans as appropriate.

• Ensure compliance with Medicare Part A and Part B documentation requirements, PDPM guidelines, and payer-specific regulations.

• Maintain productivity standards established by the rehabilitation department.

• Follow infection prevention, OSHA, HIPAA, and facility safety policies.

• Supervise Occupational Therapy Assistants (COTAs) and rehabilitation aides as permitted by state law and facility policy.

• Participate in quality improvement initiatives and continuing education activities.

• Perform other duties as assigned.

• Graduate of an accredited Occupational Therapy program.

• Current, active Florida Occupational Therapist license in good standing.

• Current CPR/BLS certification (or ability to obtain upon hire).

• Experience in a skilled nursing facility, long-term care, rehabilitation, or geriatric setting preferred.

• New graduates are encouraged to apply where appropriate.

• Knowledge of occupational therapy principles, evaluation techniques, and treatment interventions.

• Understanding of Medicare, Medicaid, PDPM, and rehabilitation documentation requirements.

• Strong clinical assessment and problem-solving skills.

• Excellent verbal and written communication abilities.

• Ability to work effectively within an interdisciplinary team.

• Strong organizational and time management skills.

• Ability to maintain resident confidentiality in accordance with HIPAA.

• Ability to lift, transfer, and assist residents in accordance with safe patient handling procedures.

• Ability to stand, walk, bend, kneel, reach, and perform repetitive movements throughout the workday.

• Ability to occasionally lift up to 50 pounds with assistance when appropriate.

• Visual acuity and manual dexterity sufficient to perform therapy treatments and documentation.

• Skilled nursing and rehabilitation environment.

• Frequent interaction with elderly residents, family members, physicians, and healthcare professionals.

• Exposure to infectious diseases and bodily fluids requiring adherence to standard precautions and personal protective equipment (PPE).
