We operate more than 90 sites across San Diego County, including multiple clinics and programs that serve a diverse patient population. Job Responsibilities Serves as a liaison between internal and external customers, including patients/clients, other FHCSD service staff, and external service providers, to improve patient/client quality of care and flow. Completes all required documentation accurately and on time, ensuring compliance with department standards, grant funder, and insurance requirements. Collaborates with healthcare and departmental program members on retention, appointment completion, and identifying barriers to care through quality improvement projects and case reviews. Engages in patient/client interactions via telephone and in person to identify and meet needs, including facilitating referrals, resources, and follow‑up. Assists with scheduling, organizing, and managing a patient/client’s healthcare maintenance and treatment. Performs general clerical tasks such as filing, processing paperwork, data entry, scanning, agenda creation, minutes, and encounter reports. Uses discretion to answer routine and complex questions, forwarding appropriate issues. Handles confidential information and performs additional duties as assigned. Qualifications and Requirements High School diploma or GED required. 2+ years of experience in health education or a related medical/health field preferred, or equivalent combination of education and experience that provides the necessary skills. Specific certifications may be required or preferred (e.g., CA HIV Test Counselor, limited CPT certification for waived testing). Traveling between sites is required; must have a vehicle, valid California driver’s license, proof of minimum insurance, and a satisfactory driving record. Demonstrated ability to maintain positive working relationships with employees and departments. Intermediate interpersonal, customer service, and communication skills. Ability to communicate effectively with patients and families who speak English as a second language. Strong cultural sensitivity, discretion, and confidentiality handling. Ability to work independently and in a team‑based environment. Basic computer literacy for electronic medical record documentation and related tasks. Knowledge of health and social service systems in San Diego County preferred. Bilingual in English/Spanish preferred or required depending on assignment. Compensation and Benefits Regular full‑time, Monday through Friday, 8:30 a.m.–5:00 p.m. or 8:00 a.m.–4:30 p.m. on site at 7146–7152 Broadway, Lemon Grove, CA 91945.
