Family Nurse Practitioner or Physician Assistant Per Diem/Part Time (Antioch, CA)

Position Summary The Family Nurse Practitioner (FNP) or Physician Assistant (PA) will be responsible for direct patient care and other administrative duties as designated by the Director of Primary Care & Quality. The Medical Doctor will provide overall direction for all patient care activities, and the Director of Primary Care & Quality will provide administrative supervision. Qualifications A valid license to practice as a Registered Family Nurse Practitioner (FNP) or Physician Assistant (PA) in the State of California, National Licensure for Nurse Practitioner (FNP-C) or National Physician Assistant license (PA-C), with the ability to sign prescriptions and furnish medications. Master’s degree or higher. A certificate of completion from an accredited Nurse Practitioner program or Physician Assistant Program. Board certification requirement. Current Basic Life Support (BLS) certification. Current DEA certification. Valid California driver’s license. Proof of auto insurance. Knowledge of appropriate patient care rationale. Ability to maintain positive interpersonal relationships with co‑workers and health team members. Proficiency with Microsoft Office, Microsoft Access, data entry, and business email. Passed physical exam and TB test. Responsibilities Provide primary care, including patient history, physical examinations, diagnosis, and treatment plans for primary, secondary, and tertiary services in accordance with standardized protocols for Nurse Practitioners and Physician Assistants. Communicate with patients or guardians about care plans, diagnostic results, and answer questions; deliver health education and disease‑specific information; coordinate referrals to specialized care. Review patient records for allergies, problems, medications, and immunization status. Order or execute tests, analyses, and diagnostic imaging. Analyze test results, diagnoses patient conditions, administer or prescribe treatments, determine and prescribe medications with dosage and schedule. Prescribe vaccinations to immunize patients from communicable diseases. Provide prenatal, postnatal, and other patient care as required. Perform minor surgical services such as toenail removal, mole and cyst removal, foreign object removal; suturing; general orthopedic evaluations; basic rehabilitation for musculoskeletal injuries; general internal medicine, family medicine, and pediatrics; general dermatology; general eye problems; general EKG interpretation; emergency management; general gynecologic expertise. Communicate with the Medical Doctor regarding quality care and treatment issues. Conduct clinical chart review studies to assess and enhance patient outcomes. Serve as a resource for community agencies. Use technology tools for research, data entry, record keeping, and business communication via email. Meet all clinical obligations of the medical practice, including accrediting agency compliance, CLIA compliance, drug prescription compliance, HIPAA, state and federal regulations, and safety regulations. Follow all internal policies and procedures; complete all operational tasks by departmental guidelines to ensure appropriate clinical processing and documentation. See patients in a timely and efficient manner, maintaining scheduled appointments in family practice throughout the day. Be accountable for proper coding and appropriate charging for all services rendered to maintain financial viability. Chart in the patient’s medical record within 48 hours of the visit and maintain confidential records. Report to the Director of Primary Care & Quality on issues related to the functioning of BBFHC. Drive to other work sites for meetings, conferences, or other functions. Administrative Job Functions Show ongoing support and respect for administration in all communications. Address administrative issues directly and privately with those concerned. Manage time by seeing patients within allotted time, adjusting to daily patient flow to remain on schedule. Maintain productivity by seeing a minimum caseload of 18–20 patients per 8‑hour shift, meeting variable daily circumstances and emergency care. Report to the Medical Doctor and designated designees, including the CEO. Complete patient charts and submit medical bills daily. Be flexible, adapt to change, and model professional behavior; think creatively to provide solutions rather than just identify problems. Comply with all HIPAA rules and regulations.
