Licensed Practical Nurse II - Shift Supervisor - On Grounds Program

The Department of Mental Health (DMH) is seeking an exceptional Licensed Practical Nurse for this full-time position at the Southeast Area On Grounds Program to work as a shift supervisor and direct service provider in a residential setting while maintaining a person centered, trauma informed atmosphere and professional performance. This position requires transportation of clients to appointments and other community activities. Position details: 40 hours per week. The selected candidate will work the first shift from 6:45am to 3:15pm. The scheduled days off are Friday and Saturday. Employees in this position will be required to perform mandatory overtime. Duties and Responsibilities Ensure care and rehabilitation is provided as prescribed by the Community Service Plans (CSP). Ensure safe staffing is in place and coordinate with the Administrator on Call (AOC) when necessary. Coordinate medication services including following physician orders, prescriptions, filling out Residential Information Sheets, checking medication availability, writing self-medication check-off sheets, and posting medication changes. Monitor, report, and record client responses to medication and treatment regimens and report findings to the supervisor for follow-up with appropriate medical practitioner. Educate clients and staff about medications and their effects through in-service training and program resources. Administer medications and treatment orders as prescribed by an appropriate medical practitioner. Accompany clients and ensure they receive emergency care as needed. Maintain and monitor a medication closet, first aid, and infection control supplies to ensure everything is labeled, kept in order, and that supplies are adequate and not expired. Required Qualifications Valid license as a practical nurse under the Massachusetts Board of Registration in Nursing. Valid Massachusetts Class D Driver’s License or equivalent. Preferred Qualifications Experience working in a Community Mental Health Setting. Bilingual or multilingual fluency. About the On Grounds Program On Grounds Program is part of the Taunton/Attleboro Site that transitions clients from an inpatient setting to a more independent community living setting. The program is staffed 24 hours a day, 7 days a week, and aims to create an environment which promotes growth and independence for patients, families, and employees alike.
