Duties and Responsibilities:

• Perform initial and ongoing clinical assessments to identify patient needs.
• Develop and revise a Plan of Care in consultation with the physician and care team.
• Provide therapeutic treatments and evaluate equipment needs.
• Evaluate outcomes of care with appropriate discharge planning.
• Apply infection control concepts to prevent contamination.
• Supervise PTA and HHA as appropriate.
• Participate in mandatory continuing education and in-service programs.
• Wound care management.

Required Skills/Abilities:

• Strong therapy skills based on accepted standards of practice.
• Excellent interpersonal skills.
• Knowledge of durable medical equipment.
• Ability to work a flexible schedule and travel locally.
• Reliable transportation and valid auto liability insurance.

Required Education and Experience:

• Bachelor of Science in Physical Therapy from an accredited program.
• Current Texas State License as a Physical Therapist.
• Two years' experience in an acute care or rehabilitation setting.

Physical Requirements:

• Must be able to lift and/or move up to 50 pounds.
• Frequent talking, standing, and hand dexterity required.
• Close vision, color vision, and depth perception abilities needed.

Work Location & Travel Requirements:

• Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
• This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
