Position Overview

The Licensed Practical Nurse (LPN) provides direct care to residents , observes and monitors health and vital signs, maintains accurate documentation, and works closely with Registered Nurses (RNs) and Unit Managers to deliver safe, high-quality care.

Key Responsibilities

  • Provide personal care and assistance with daily living activities

  • Monitor resident health and report changes to nursing staff

  • Collaborate with RNs and Unit Managers on care plans

  • Maintain accurate and timely documentation in compliance with facility and state standards

  • Support a positive, resident-focused environment for both residents and staff

Qualifications

  • Current Licensed Practical Nurse (LPN) license in Massachusetts

  • Long-term care experience preferred but not required

  • Strong communication, teamwork, and problem-solving skills

  • Compassionate, reliable, and committed to high-quality resident care
