Education/Training
Completion of an ACGME-accredited residency in Internal Medicine or Family Medicine
Board Certification
Board Eligible or Board Certified in Internal Medicine or Family Medicine
Clinical Experience
Minimum 2 years of current clinical experienceDocumentation of case logs for the previous 2 years required
Licensure
Active, unrestricted Virginia Medical LicenseActive DEA registration with Virginia address
Required Certifications
ACLS CertificationCredentialing Requirements
Immunizations
Current Flu Vaccine administered within the current flu season/year
Case Logs
Documentation of case logs for the past 2 years
CME Requirements
Proof of completed CME/CEU credits for the previous 2 years, orCME Attestation Form included with the hospital application
Facility Privileges
Must be comfortable performing privileges selected on the Delineation of Privileges formEMRCernerMalpractice CoverageRequired Limits: $2,750,000 / $8,250,000Note: Limits increase annually in July
