RN Case Manager Opportunity – Lodi, California

Location: Lodi

Position Summary

Schedule & Assignment Details

• Shift: Day Shift (8:00 AM – 6:30 PM)

• Hours: 40 hours/week

• Shift Length: 10 hours

• Days/Week: Variable (includes weekends)

• Duration: 13 weeks

What You’ll Do

• Lead coordination of patient care across interdisciplinary teams

• Monitor care plans for effectiveness, timeliness, and appropriateness

• Collaborate with physicians on treatment plans, length of stay, and discharge strategies

• Serve as a liaison between patients, families, and care providers

• Focus on complex populations (chronic conditions, high utilizers, etc.)

• Coordinate transfers and ensure continuity of care

• Support regulatory compliance and data reporting

Requirements

Education & Experience:

• BSN (preferred)

• Minimum 1 year acute care case management experience

Licensure & Certifications:

• Active RN license (required)

• Case Management Certification (preferred)
