About This Role The VNA Hospice RN Case Manager provides skilled care and case management to clients and families, ensuring adherence to professional hospice standards and promoting quality of life through direct care, teaching, and counseling. Highlights Pay: $37.01 - $53.91/Hourly Location: Northampton, MA Facility: VNA & Hospice of Cooley Dickinson, Inc. Schedule: Full-Time, Day shift What You'll Do Provide skilled care and case management to clients/families. Communicate and document observations and assessments. Promote and maintain quality of life for clients/caregivers. Utilize nursing theory and processes in care delivery. Requirements Education Graduate of an approved, accredited school of professional nursing required; BSN preferred. Experience Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred. Licenses & Certifications Current RN License in Massachusetts.
