Description

• Conduct patient assessments in inpatient and outpatient environments and create tailored treatment plans.
• Implement therapeutic interventions that enhance mobility, strength, balance, and everyday function.
• Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes.
• Guide patients and their families through therapeutic objectives, discharge processes, and essential home exercises.
• Keep thorough documentation of evaluations, treatments, and progress in line with facility policies and regulations.
• Participate in interdisciplinary care rounds and team discussions to facilitate coordinated patient care.

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution.

Certifications

• State licensure as a Physical Therapist.
• CPR certification preferred.
• Special certifications (neurological, geriatric) are advantageous but not mandatory.

Skills

• Previous experience is strongly preferred.
