Key Responsibilities

  • Conduct detailed assessments to identify the social, emotional, and psychological needs of hospice patients and their families.
  • Provide individualized and group counseling utilizing evidence-based therapeutic methodologies to help families navigate their challenges during end-of-life care.
  • Develop tailored service plans in collaboration with patients, families, and interdisciplinary teams, outlining achievable goals and objectives.
  • Serve as an advocate for clients, promoting their rights, ensuring access to necessary resources, and addressing social justice issues that affect their care.
  • Engage in program development and evaluation, including contributing to grant reporting to secure funding for community initiatives.
  • Collaborate with community organizations, healthcare providers, and volunteers to enhance available services and ensure comprehensive patient care.
  • Participate constructively in team meetings, ongoing training, and various community outreach activities to enhance social support networks.

Patient Population / Client Focus

The primary focus of this role is on patients receiving hospice care—individuals who are dealing with end-of-life issues, chronic pain, and complex family dynamics exacerbated by grief and loss. As a vital team member, you will provide emotional support, facilitate family meetings, and ensure that care approaches are aligned with the cultural and personal values of patients and their families.

Qualifications

  • A Master's degree in Social Work from an accredited institution is required.
  • Possession of a current and active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Demonstrated knowledge of and skills in social justice, trauma-informed care, and advocacy strategies.
  • Excellent communication and interpersonal skills to effectively engage with patients, families, and interdisciplinary team members.
  • Exceptional organizational skills and the ability to manage multiple cases and priorities effectively.

Preferred Experience

  • A minimum of 2 years of experience in a clinical or nonprofit setting focused on social work, counseling, or mental health services is preferred.
  • Experience working within hospice or palliative care is a significant advantage, given the unique challenges and sensitivities of this client population.
  • Familiarity with community resources and support systems in New Jersey will facilitate service delivery and client support.
