Shifts: Monday - Friday 8am-5pm, on call every 3rd weekend and 1-2 nights during the week

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers for individualized care planning.
• Address and reassess cost-efficient levels of care.
• Implement non-medical support to ensure treatment compliance.
• Coordinate care transitions as needed.
• Work with physicians and care teams for patient care transitioning.
• Engage specialty resources for optimal resolution.
• Keep detailed records of outcomes during management.
• Identify opportunities for health promotion and illness prevention.
• Act as a patient advocate by protecting privacy and confidentiality.

Requirements of the RN - Case Management

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Ability to analyze complex medical information and make rational decisions.
• Case Management certification.
• KY RN- compact license.
