Physical Therapist

Provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. The qualified therapist assists in evaluating level of function and helps develop the plan of care, revising it as necessary. Prepares clinical and progress notes, advises and consults with family and other personnel, and participates in in-service programs. Participates and assures that services are consistent with the organization's vision, mission, values, and strategic goals. Promotes an effective working relationship and works as part of the team to meet organizational goals and objectives. Utilizes resources, supplies, and time efficiently to increase the value of services to the community. Promotes positive, supportive, respectful communication to all external sources, patients, families, and staff. Displays behaviors in support of the overall organization. Supports public relations and marketing initiatives to the local community. Strives for continuous improvement in all aspects of the Physical Therapist function and care delivery process. Develops and maintains a customer-oriented environment responsive to the needs of all customers. Develops and revises plans of treatment and discharge plans in conjunction with patients, families, and third-party payors. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiates appropriate rehabilitative programs. Provides supervision of assistants to ensure patient health, safety, and compliance with the plan of care. Educates patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies. Implements home therapy programs, including gait training, transfers, activities of daily living, home modifications, and care. Evaluates patient progress periodically and makes necessary adjustments to physical therapy programs. Instructs para-professional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, and home exercise programs. Directs, coordinates, evaluates, and supervises the quality of patient care services provided, including supervision of aides. Interacts with patients, physicians, and referral sources in a manner conducive to positive relationships. Displays a high level of patience, courtesy, and tact. Supports case management philosophy, including collaborative customer-focused planning. Participates in regularly scheduled patient conferences to discuss patient care and ensure coordinated plans of treatment. Assists in the orientation of new rehabilitation personnel. Completes all appropriate clinical records for compliance with legislation and delivery of seamless service. Maintains current knowledge of available community-based services. Demonstrates flexibility, enthusiasm, and willingness to cooperate while working with customers and others. Resolves problems efficiently at the point of contact. Makes recommendations regarding ways to improve quality of patient care and customer service. Ensures that patients' needs are met through the care delivery process. Ensures that all necessary authorizations and information release forms are signed by patients or responsible parties. Participates in professional growth, learning opportunities, and shares acquired knowledge with team members. Responsible for key performance indicators established by management. Monitors results and takes corrective action on quality indicators, including levels of customer satisfaction. Understands the impact of the Physical Therapist function on patients and other care providers. Pursues efforts to reduce avoidable costs and errors. Develops quality plans of care consistent with care paths and complies with regulatory practices. Participates in education and training programs as needed.
