Job Title: Registered Nurse (RN) Case Manager Home Health | Clinical Autonomy & Leadership
Company: Home Health Care, Inc.
Location: Greater Twin Cities Metro Area (21-county service area)
Schedule: Full-Time, Monday to Friday, 8:00 AM 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 $130,000 annually plus $6,000 in first-year bonuses

What You'll Do
Lead comprehensive nursing assessments and care planning.
Manage a home health caseload across homes and community settings.
Perform OASIS visits and coordinate interdisciplinary care.
Provide education, guidance, and clinical direction to patients and families.
Collaborate with physicians and care teams to drive outcomes.
Maintain accurate, compliant documentation.
What We're Looking For
Active Minnesota Registered Nurse (RN) license.
3+ years of RN experience, home health preferred.
Strong clinical decision-making and assessment skills.
Confidence working independently in a field-based role.
Organized, accountable, and patient-focused.
Reliable transportation.
