Hospice Registered Nurse Case Manager

Role Description The Hospice Registered Nurse Case Manager is a full-time, hybrid role based in Birmingham, AL, with some work-from-home flexibility. This role is responsible for coordinating and delivering hospice nursing care, including comprehensive assessments, care planning, and ongoing evaluation of patient needs. The Case Manager collaborates with an interdisciplinary team to manage symptoms, provide education to patients and families, and ensure safe and appropriate transitions of care. Day-to-day responsibilities include managing a caseload of hospice patients, overseeing medication and treatment plans, documenting clinical services, and participating in case conferences. The role also involves supporting family members, advocating for patient preferences, and promoting quality, compliance, and continuity of care. Qualifications Strong clinical Nursing and Medicine skills to provide high-quality hospice care and symptom management. Experience in Case Management to coordinate interdisciplinary care, manage caseloads, and support continuity of services. Knowledge of Discharge Planning to ensure safe transitions between care settings and comprehensive follow-up plans. Familiarity with Utilization Management to apply appropriate levels of care and adhere to regulatory and payer requirements. Active Registered Nurse (RN) license in Alabama; hospice or home health experience preferred. Excellent communication, documentation, and interpersonal skills to work effectively with patients, families, and care teams. Ability to work in a hybrid environment, manage time independently, and travel locally to patient homes as needed.
