Physical Therapist

The Physical Therapist 1 is responsible for evaluating/assessing the needs of referred patients, formulating treatment plans, providing therapy services as outlined in those plans, and collaborating with physicians, case managers, and other healthcare team members. The role requires accurate documentation of therapy services in the medical record and may include supervising physical therapy assistants, therapy techs, and students. Responsibilities Patient Care: This will have been satisfactorily performed when… Evaluation and treatment methods appropriate to diagnosis, age group, and patient’s physical and emotional status are performed and accurately documented; appropriate treatment plan is formulated as evidenced by chart review. Patient care is documented daily using the approved hospital and department documentation system, reflecting patient response to treatment, actual performance, and treatment summaries if needed (as evidenced by accurate & timely chart reviews). Daily patient charges are completed accurately, recorded in treatment documentation, on the daily charge sheet of the department, and entered in the appropriate billing system per unit, 100% of the time. Measurable short- and long‑term goals for each patient evaluated are identified and documented as evidence through chart review. Patient and family/caregiver education concerning treatment plan, expected reactions to treatment, safe use of equipment, home programs, and follow‑up care is accurately documented 100% of the time. Discharge notes that include a summary of the course of treatment, progress toward goals, discharge needs, recommendations for follow‑up, and patient or family education are accurately completed 100% of the time (as evidenced by chart review). Regulatory/Quality: This will have been satisfactorily performed when… Actively contributes to educational efforts of the department (in‑services, students/new‑hire orientation, QI); at least one in‑service per year, documentation on competencies for students and new hires, and QI data collection committee participation as assigned. Serves as a clinical instructor for Physical Therapy students on clinical internship, providing mentoring, supervision, timely feedback, and written evaluation of the student’s skills (at least twice during the internship). Effectively follows and participates in scheduling functions by promptly accommodating patients for treatment as ordered and effectively utilizing support staff for maximum productivity and fair distribution as needed.
