Family Coordinator Program/Location: Plymouth Inpatient Campus - Section 35 Unit Education/Licensure: Associate's degree, High School diploma, or GED preferred Pay Range: $22-$27/hour (experience dependent) Status: Full Time About Us High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Our goal is to help individuals and families achieve personal change and improve their quality of life. High Point's Plymouth Inpatient campus offers Section 35 services for individuals that have been civilly committed to Substance Use Treatment. These services also include acute treatment services (ATS-detoxification) and clinical stabilization services (CSS). These programs are staff secured units and not locked. Family Coordinator Requirements
- Must present as courteous and professional at all times.
- Demonstrated flexibility and ability to perform multiple tasks.
- Must have excellent verbal and written communication skills, including the
- TB/Flu and Covid-19 Assessment.
- Completion of the HPAO approved de-escalation program.
- Successful completion of all trainings upon hire and annually as required.
- Maintains assigned caseload.
- Providing direct care to clients - including assessments, individual
- Making referrals when appropriate.
- Responsible for each assigned patients' medical records. All records are to
- Communicating/involving family in treatment at patient's request.
- Maintain professionalism when connecting with individual's collaterals.
- Attend and participate in all unit, facility meetings, case conferences,
- Utilize walkie-talkie as campus responders.
- Respond to codes as necessary.
- Coordination of aftercare services, appointments, and referrals.
- Facilitation of identified groups and milieu activities as assigned.
- Participation in MDT meetings to support patient care coordination.
- Assistance with monitoring patient observation checks, as needed.
- Completion of required documentation, including: consents/releases, Group
- Attendance in supervision and staff trainings.
- Request extensions after 90 days of treatment if needed.
- High School Diploma or GED, preferred.
- Associate's degree in human services, preferred.
- One-year experience in substance use treatment and/or mental health field.
- CPR Certification, preferred.
- Personal lived experience in mental health and/or substance use recovery.
