Position Overview The Licensed Practical Nurse is responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times. Benefit eligibility depends on employment status and location. Essential Job Functions Deliver quality patient care in compliance with physician orders under direction of a Registered Nurse teammate. Perform the continuum of hands‑on client care through documentation and timely communication with the client’s physician and other caregivers. Recognize changes in patient needs and responses requiring intervention and provide care to prevent or reduce risk. Educate clients and their family members based on the client’s specific needs. Provide care in patients’ homes using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes, etc. Other Skills/Abilities Attention to detail. Time management. Effective problem-solving and conflict resolution. Good organization and communication skills. Physical Requirements Must be able to speak, write, read, and understand English. Must be able to travel. Must be able to lift 50 pounds. Prolonged walking, standing, bending, kneeling, reaching, twisting. Must be able to sit and climb stairs. Must have visual and hearing acuity. Must have strong sense of smell and touch. Must be able to reposition patients and move equipment without assistance. Must respond physically and mentally to emergency situations in the home or during transport. Environment Must function in varied environments that may involve exposure to allergens and other conditions. Possible exposure to blood, bodily fluids, and infectious diseases. Qualifications Active LPN License in the state of application. Current CPR certification (hands‑on component). TB skin test current within last 12 months. Reliable transportation. Ability to read and write English. Preferred Medicare Skilled Nursing experience. Basic understanding of Oasis. 1‑year LPN experience in a home health or health care setting.
