Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapist license is required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Home health experience preferred. Candidates with prior experience conducting evaluations, developing home exercise programs, and delivering skilled in-home interventions will be given preference.
• Experience with common home health documentation systems and electronic medical records preferred.
• Ability to travel within an assigned geographic caseload and reliably provide patient visits in home settings.
• Reliable vehicle and valid driver’s license recommended for travel between patient homes.
