We are looking for an experienced Home Health RN to support the Rockford, IL area and surrounding communities on a PRN basis. Prior home health experience is required for this position.

As a Registered Nurse Home Health , you'll contribute to our success in the following ways:

• Assessing patients to determine their care needs.

• Completing initial and ongoing patient assessments.

• Protecting patient and staff privacy in accordance with HIPAA.

• Providing nursing care according to the plan of care and physician orders.

• Monitoring patient conditions and reporting changes to the care team.

• Educating patients and families on their care and safety.

• Taking appropriate action when patient conditions change.

• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.

• 1+ year of experience as a nurse in a clinical care setting.

• Home Health experience is preferred, but not required.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $55 dollars per point based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
