Clinical Health Facility Surveyor III

Position Description The Oklahoma State Department of Health (OSDH) seeks a Clinical Health Facility Surveyor III to perform technical licensure, certification surveys, and complaint investigations for a variety of health facilities. The Surveyor evaluates care provided by licensed or registered staff, identifies deficiencies, provides consultative assistance, and ensures compliance with federal and state laws. Responsibilities Apply professional standards to observe and evaluate nursing, dietary, social, laboratory, pharmaceutical, respiratory, speech, physical, and occupational therapy services. Conduct entrance and exit interviews with facility administrators and staff, review compliance history, and tour facilities. Sample residents/patients/clients and interview them, their families, and facility personnel to document care and respond to complaints. Review administrative records including policies, staffing logs, in‑service programs, and medical records for regulatory compliance. Complete required forms and reports detailing survey findings and deficiencies. Investigate abuse, neglect, and immediate jeopardy situations; make determinations and recommend actions regarding certification or licensure. Provide technical assistance and consult with facilities to improve compliance. Present findings at hearings, court proceedings, or in‑person briefings. Qualifications Medical Technologist : Registration with the American Society of Clinical Pathologists and six years’ experience as a Registered Medical Technologist. Medical Practitioner : Bachelor’s degree, valid Oklahoma medical license, and six years’ professional experience. Practical Nurse : Valid Oklahoma practical nursing license and eight years’ experience in a healthcare setting. Registered Nurse : Valid Oklahoma RN license (or eNLC), seven years’ experience, or a bachelor’s degree in nursing with six years of experience. Social Worker : Valid Oklahoma social work license and six years’ professional experience. A background check and legal authorization to work in the United States are required. Certifications & Training Within one year of hire, the Surveyor must complete and obtain certification for Hospice & Home Health, Hospital, Ambulatory Surgery Center, and EMTALA. Knowledge, Skills & Abilities Knowledge of professional practice and ethics specific to the Surveyor’s licensure. Understanding of federal and state health‑care regulations and licensing statutes. Interviewing techniques, including memory retrieval methods. Strong written and oral communication skills for reporting findings. Judgment and decision‑making in complex, potentially hostile environments. Ability to lead and coach a team of surveyors. Showcase of empathy and professionalism when interviewing residents/patients or families. Physical Demands & Work Environment Primarily office‑based with periods of standing, walking, lifting, and reaching. Survey activities may require extended hours, off‑hour, weekend, or evening work.
