Physical Therapist | Geriatrics

We're working on behalf of a healthcare organization whose PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. The organization’s innovative and comprehensive range of medical services to participants is what ignites their passion to treat the whole person and not the symptoms. They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to their values, team culture, and mission. We seek candidates with two (2) or more years of experience with the frail and/or elderly poluation for the role of Physical Therapist . Responsibilities Conduct Physical Therapy assessments in the facility and in the home to define the Physical Therapy care needs, preferences, and goals of the participants. Actively participate in IDT meetings to develop participant care plans. Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans. Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity. Supervise the Therapy Assistants, including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP). Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment. Qualifications Bachelor’s degree in Physical Therapy from an accredited program and hold valid California PT License, or proof of License eligibility. Master’s Degree in Physical Therapy or Doctorate in Physical Therapy preferred. At least two (2) years of OTA experience in a clinical setting. Requisite knowledge to treat frail, elderly participants with complex conditions. Benefits Competitive salary up to $131,000 based on experience.
