Job Description & Responsibilities Under the supervision of the Nurse Consultant (Supervisor) and with clinical guidance from the Supervising Registered Nurse II, the LVN assists in implementing the CalAIM for Justice Involved Initiative, Whole Person Care Program, Complete Care Model, and other clinical initiatives. The LVN collaborates with the healthcare team and internal/external stakeholders to ensure timely delivery of pre‑release health care services for patients 90 days or less from release. The LVN’s duties include addressing pre‑release needs of patients in the eligible population. Minimum Qualifications Possession of a valid, active license from the State of California to practice as a Licensed Vocational Nurse.
