Job Description Full-time Intake Nurse (Hybrid schedule, Philadelphia, PA) Position Responsibilities: Answer phones, receive all referral and intake information. Complete referral/intake forms and route them for admission approval. Assist in scheduling appropriately skilled personnel and enter scheduling information into the computer. Check common working files for Medicare coverage and verify benefit period. Perform other duties as directed. Job Conditions: Office setting requiring regular phone usage, sitting, stooping, and lifting of office supplies (e.g., printer paper stacks). Ability to utilize PC, calculator, multi-line telephone, and other related office equipment. Qualifications: High school graduate or GED. LPN license in the state of Pennsylvania. Experience in a Home Health Care or Hospice setting preferred. Computer literate and able to maintain simple records. Effective oral and written communication skills. At least 3 years of customer service experience. At least 3 years of EMR system experience.
