Case Worker (LMSW / LMHC) – Veterans Shelter Program | Northport, NY | Full-Time

What You’ll Do

• Conduct assessments and create personalized Independent Living Plans (ILPs)
• Connect Veterans with community resources, recovery programs, and supportive services
• Collaborate with VA partners, program staff, and leadership to ensure holistic care
• Respond to urgent housing needs and provide on-call support as part of a dedicated team

Who You Are

• Licensed LMSW or LMHC with experience in case management
• Familiar with working with homeless populations, residential programs, or those with substance use and mental health challenges
