  • Graduate of an accredited school of professional nursing and licensed to practice as a Wound Care Registered Nurse in the State of California. BSN preferred.
  • Current American Heart Provider Basic Life Support (BLS) Certification from The American Heart Association or The American Red Cross for Healthcare Providers
  • Must possess qualifications for Medical/Surgical RN
  • Certification in wound care education such as WCC, WTA, or CWOCN
  • Charge Nurse experience, preferred
  • Minimum 2 years professional nursing experience

Open Until Filled. This is a Part-Time position, and the hours for this position range from 48-55 hrs. per two week pay period.

Job Summary


The Wound Care RN may be required to work in Medical-Surgical units, Emergency Department, Critical Care Units, Obstetrics department and Clinic Management.


Duties

  • Provide excellent nursing care to patients suffering from wounds.
  • Assess and evaluate patients with wounds and injuries.
  • Obtain cultures to assess wounds and injuries.
  • Evaluate wounds and injuries for infections or other illness factors that cause wounds.
  • Initiate nursing care procedures in managing chronic, acute and traumatic wounds.
  • Coordinate with discharge planning, physicians, specialty services, home health, and outpatient services in managing wounds.
  • Ensure optimum patient care delivery in wound care nursing procedures.
  • Educate and counsel patients and their families on wound care processes and issues.
  • Demonstrate wound care procedures to other caregivers.
  • Sanitize and maintain the premises of wound care neat, clean and hygienic.
  • Communicates with Nurse Managers of Nursing Services, other department directors and the hospital Administrator regarding problems, interventions or decisions made by Wound Care RN.
  • Assesses emergency situations such as bomb threat, earthquake, fire, etc.
  • Maintains current knowledge of situations which may affect overall functioning of the hospital
  • Functions as patient advocate
  • Maintains current knowledge regarding patient status
  • Assists with direct patient care as needed
  • Actively participates in Quality Assurance/Improvement Program
  • Monitor wound vacuum therapy in cooperation with surgery team or overseeing physician and participate in dressing changes as needed we well as educate and assist bedside nurse in care
  • Appropriately assesses patients and determines need for intervention
  • Provides access to supplies, equipment and pharmaceuticals when those departments are closed
  • Maintains a high level of mental and emotional tolerance and an even temperament when dealing with people
  • Provide ostomy care and education to patients under guidance of WOC nurse
  • Consult with WOC nurse regarding questions or concerns and collaborate in developing patient care plans as appropriate
  • Participate in staff education activities including but not limited to informal 1:1 education, staff meetings and skills fairs
  • Participates in special projects as assigned by Vice President of Nursing
  • Accepts responsibility for care and outcomes
Assists Physician/Surgery Team in rounds with Patients as Requested

  • Devises treatment plan for patients with acute and chronic wounds
  • Records, measures, documents wounds and makes recommendations for treatment plan in EMR "Wound Photo Documentation" template
  • To make a positive and significant contribution in the working environment by providing excellent nursing care to patients suffering from pressure injuries and other acute and chronic wounds.
  • Responsible for providing effective and competent wound care management to the patients.
Principal Duties


Responsibilities:

  • To provide effective wound care management to patients by:
  • Managing and coordinating the overall activities related to wound care in accordance to the hospital objectives and policies.
  • Plan activities to manage wound care in the assigned unit.
Basic Functions


Submits weekly reports to the department head as requested, regarding wound care assessments of each patient.

  • To disseminate and document wound care policies and procedures.
  • Collaborate and coordinate with the head nurses and wound care links in the standardization of methods, policies and procedures which will affect delivery of patient's wound care.
  • Review the written and existent policies and procedures concerning wound care management and communicate to all concerned as well as involved department.
  • Manage nursing unit functions in terms of equipment, supplies and support services related to wound care.
  • Assess, evaluate and coordinate the supplies for dressings needed to provide optimal wound care.
  • Assist and coordinate with the department head and wound care links the laboratory result especially the wound culture results needed to be reported to the attending physician and infection control team
  • See to it that the nursing units by the help of the wound care link assigned, is totally equipped and maintained with adequate dressing supplies to the need of patients.
  • Supervise the staff and review functions and services.
  • Direct, coach, train and delegate activities with the staff regarding wound care management; includes BRADEN SCALE (monitoring patient risk to have the pressure pressure injury), proper wound care assessment, and proper dressing techniques.
  • Coordinate individual activities for more relevant, less redundant result in order to maintain a harmonious relationship, effective and open communication to the staff.
  • Coordinate the standardized nursing care plan based on disease entities, which reflected in the patient's medical record for verification, follow up and consultation.
  • See to it that medical advices and health teachings are properly imparted to the patients as well to the relatives.
  • Assist nurses in interpreting and implementing policies and procedures of wound care management in the units.
  • Guide, counsel and evaluate staff in executing their responsibilities and duties as defined by the job description and performance competencies in relation to wound care management and safety requirements.
  • Motivate nurses by the help of the nursing educator toward increased effort in nursing research, which will give comprehensive wound care management.
  • Promote the development of nurses' maximum capacity through education, experience, supervision and evaluation.
  • Involve self in orienting new staff (staff nurses) in the hospital set-up, policies and procedures.
  • Implement health maintenance wound care programs.
  • Attend conferences, seminars with paramedical team for efficient delivery of standard patient's wound care management.
  • Plan and implement nursing updates to ascertain preparedness of the team.
  • Consultation - acts as a consultant on wound care problems related to various clinical areas, and recommends appropriate procedures and techniques to prevent pressure injuries development. Promptly respond to consults and requests for new patient evaluations or re-evaluations.
  • Surveillance - re-evaluate each patient under wound/ostomy services at least weekly and more often as appropriate
  • Resource - provide reference for specific wound related to hospital pressure injury and prevention.
  • Teaching - involves in formal and informal teaching of personnel, patients and watchers.
  • Research - initiates and participates in studies relevant to wound care management.
  • Documentation, Recording and Reporting - writes and provides reports as requested to be submitted to the departmental head, and this also serves as your basis or evidence that you have provided proper wound care management for your patient.

Work area is well lighted and ventilated. Regularly exposed to the risks of blood borne and airborne diseases. Contact with patients in a wide variety of circumstances. Exposed to noise, chemicals, communicable diseases, and hazardous materials. Wears personal protective equipment as required.


While performing the duties of this job, the employee is regularly required to stand, use hands to handle or feel objects, or controls; each with hands and arms; and talk to hear. The employee is frequently required to walk or stand for extended periods, occasionally required to sit, balance; stoop, kneel or crouch.


The employee must frequently lift and/or move 50 pounds and occasionally lift and/or move up to 100 pounds with help.
