Responsibilities Conduct comprehensive assessments and ongoing re‑assessments of residents—including psychosocial, physical, mental health, environmental, and spiritual needs. Write comprehensive assessments and develop client‑centered Community Living Plans based on assessment information. Arrange and monitor appropriate community services based on knowledge of Medicare, Medi‑Cal, and other entitlement programs. Establish and maintain a care management relationship with clients and their significant others, offering respect, dignity, and support. Provide crisis intervention, advocacy, problem solving, and therapeutic interventions. Meet with clients at least monthly and more often as needed; review and modify their Community Living Plan on an ongoing basis. Document case management activity via progress notes within 24 hours; add new problems to the service plan as needed. Maintain required paperwork and follow a clear, concise, and consistent charting system for continuity of care. Educate clients and significant others about resources and, when possible, train them to provide their own case management. Establish and maintain open communication with community providers, including physicians and other health care and social service workers. Provide appropriate information on individual client care and program operations while maintaining confidentiality. Monitor the quantity and quality of services provided by other providers. Terminate clients when appropriate and document the process as required, in collaboration with the client, caregiver, and services. Participate in research studies and data collection, as required. Participate in and promote continuous quality improvement efforts. Attend and actively participate in IOA and Santa Clara Methodist Retirement Foundation team and program meetings, activities, and problem‑solving endeavors; contribute to open lines of communication within the teams. Utilize supervision appropriately, maintaining open communication and providing updates on caseload activity. Incorporate the ethical standards of the National Association of Social Workers into all interactions. Understand and apply regulatory and procedural requirements of Community Living Services and IOA’s policies and procedures. Attend continuing education classes and/or in‑service training to enhance knowledge related to case management, gerontology, and relevant areas. Perform all other reasonably related responsibilities as assigned. Education B.A. or B.S. in Social Work or related field required. Background and Experience One year of experience working with disabled adults and/or frail elderly required; experience with the medical and psychosocial problems of functionally impaired adults and the frail elderly. Demonstrated case management skills and experience in the community health care delivery system. Detail oriented with good problem‑solving skills and the ability to prioritize multiple tasks. Computer literacy required. Bilingual highly preferred: English plus Cantonese and/or Mandarin and/or Spanish. Exceptional communication and presentation skills when working with frail elderly, functionally impaired adults, their support systems, and health professional teams. Physical & Language Requirements Ability to lift up to 25 lbs. Ability to reach, bend, and walk. Five‑finger manipulation. Ability to speak and write English clearly. Compensation Compensation Range: $71,797-$85,808 per annum. This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is dependent on actual experience, education, and other factors.
