Duties

• Conduct comprehensive patient evaluations to determine physical therapy needs, utilizing medical terminology and anatomy expertise.

• Develop and execute individualized care plans tailored to each patient's condition, including orthopedics, geriatrics, pediatrics, and post-surgical rehabilitation.

• Deliver hands-on therapy techniques such as trigger point therapy and other physiotherapy modalities to alleviate pain and restore function.

• Collaborate with interdisciplinary teams on discharge planning, acute care interventions, and home & community care strategies.

• Monitor patient progress regularly, adjusting treatment plans accordingly while documenting outcomes in EMR (Electronic Medical Records) systems.

• Educate patients and their families on exercises, injury prevention, and self-care techniques for home & outpatient settings.

• Assist with inpatient hospital care, outpatient clinics, home health visits, and outpatient therapy sessions to ensure seamless patient care continuity.

Requirements

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.

• Proven experience in inpatient hospital environments, outpatient clinics, or home health settings with a focus on patient care.

• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy.

• Familiarity with EMR systems for accurate documentation of care plans and patient progress.

• Experience working with diverse populations such as pediatrics, geriatrics, orthopedics, and those requiring acute or chronic care management.

• Ability to perform discharge planning effectively while coordinating with healthcare teams for optimal patient outcomes.
