This position works closely with residents, families, responsible parties, facility leadership, and the interdisciplinary care team to support admissions, complete psychosocial assessments, assist with care planning, connect residents with resources, and help promote a safe, supportive, and resident-centered environment.

In this purpose-driven role, you will: The Social Worker plays an important role in supporting the mental, emotional, and social well-being of residents in a skilled nursing facility. This position works closely with residents, families, responsible parties, facility leadership, and the interdisciplinary care team to support admissions, complete psychosocial assessments, assist with care planning, connect residents with resources, and help promote a safe, supportive, and resident-centered environment.
Key Responsibilities Include:
- Build positive relationships with residents and responsible parties to support admissions, counseling, care planning, and identification of resident needs.
- Complete initial, routine, and as-needed psychosocial and socialization assessments.
- Support residents with adjustment, social interaction, reality orientation, intellectual stimulation, and emotional well-being.
- Assist with care plans, acute care plans, care plan meetings, and interdisciplinary communication.
- Discuss advance care planning with residents and responsible parties upon admission and as required by state regulations.
- Refer residents to appropriate community resources and supportive services, including vision, dental, hearing, and psychosocial services when needed.
- Assist the Administrator with resident, family, and responsible party concerns, including follow-up and resolution support.
- Support room changes by communicating with residents and responsible parties and completing required forms.
- Perform suicide and homicide risk assessments and complete required follow-up as needed.
- Make morning rounds and provide important department updates during daily department head meetings.
- Attend required committee meetings and complete monthly QA reports.
- Assist with investigations and MDS completion as needed.
- Partner with the Business Office Manager and facility leadership to help address resident needs and promote emotional security.
- Work rotating weekend Manager on Duty schedule as assigned.
- Bachelor's or Master's degree in Social Work required.
- Active Texas Social Worker license preferred.
- License-pending candidates may be considered on a contingent basis and must obtain the required Texas Social Worker license within 6 months of hire to maintain employment.
- Experience in a skilled nursing facility, long-term care, rehabilitation, healthcare, or senior care setting preferred.
- Strong communication, organization, documentation, problem-solving, and resident advocacy skills.
- Ability to work effectively with residents, families, responsible parties, facility leadership, nursing, business office, therapy, and interdisciplinary care teams.
- Must be eligible to work in the United States.
