Position Overview:
We are seeking an Occupational Therapist to provide patient-centered rehabilitative care in a skilled nursing environment.
Key Responsibilities:
Perform OT evaluations and reassessments
Develop and implement individualized care plans
Address ADLs, mobility, and safety needs
Participate in interdisciplinary care planning
Complete clinical documentation accurately and timely
Qualifications:
Active CA OT license
SNF experience preferred
